Orforglipron: in the ATTAIN-1 trial, the first non-peptide GLP-1 pill cut body weight by an average of 11.2% over 72 weeks at the highest dose, against 2.1% on placebo, and it can be taken without meal restrictions (NEJM, 2025). Approved by the FDA as Foundayo in April 2026 and by the UK regulator in August, it is still under review at the European Medicines Agency, so patients in the EU cannot get it yet.
What is orforglipron (Foundayo) and where does its approval stand?
Orforglipron is an obesity medicine from Eli Lilly, sold as Foundayo: a once-daily tablet that activates the GLP-1 receptor like semaglutide does, but without being a protein. That difference is why it needs no mealtime rules. If you are weighing a pill against weekly injections, start with our Zepbound vs Wegovy head-to-head breakdown, which covers what is known about the injectables.
Three regulatory steps are documented. The US Food and Drug Administration approved orforglipron on 1 April 2026 for adults with obesity, or overweight with at least one weight-related condition, according to the US prescribing information and coverage of the launch. The UK followed: the UK government announced the MHRA authorisation on 10 August 2026, describing it as the first in Europe. In the European Union the application exists but is not concluded, as covered below.
What did the ATTAIN-1 trial show about weight loss with orforglipron?
ATTAIN-1 followed 3,127 adults with obesity and without diabetes for 72 weeks, all on a reduced-calorie diet and physical activity. On orforglipron, average weight loss was 7.5%, 8.4% or 11.2% depending on the dose, versus 2.1% on placebo. These are averages across everyone randomised, including people who stopped along the way (NEJM 2025, PMID 40960239).
| Group (72 weeks) | Average change in body weight |
|---|---|
| Orforglipron, lowest dose (6 mg) | −7.5% |
| Orforglipron, middle dose (12 mg) | −8.4% |
| Orforglipron, highest dose (36 mg) | −11.2% |
| Placebo | −2.1% |
In the highest-dose group, 54.6% of participants lost at least 10% of their weight, 36.0% at least 15% and 18.4% at least 20%, versus 12.9%, 5.9% and 2.8% on placebo. Waist circumference, systolic blood pressure, triglycerides and non-HDL cholesterol also improved. Lilly highlights a more flattering figure, 12.4% among people who stayed on treatment, per PharmExec: that is a different way of counting, one that sets aside dropouts.
Should you wait for orforglipron instead of starting an injection?
Waiting for orforglipron only makes sense where it is actually available to you and where delaying treatment is medically acceptable. In the US it is approved; in the UK it is authorised but, per the government, not currently available through the NHS while NICE evaluates it. In the EU, no marketing authorisation exists yet and no launch date has been published.
The European file for orforglipron (procedure EMEA/H/C/006632, obesity and type 2 diabetes in adults) appeared on the agenda of the EMA’s CHMP meeting of 14 to 17 September 2026 at the “list of outstanding issues” stage, the list of questions having been adopted on 21 May 2026 (CHMP agenda). The agency has not yet given an opinion. Lilly says it has applied in more than 40 countries and plans to launch shortly after each clearance (PharmExec). To see whether treatment fits your situation, you can check your BMI and weight-loss projection and take the result to your prescriber.
Can you really take orforglipron without food or water restrictions?
Yes, according to the labels: the US prescribing information says orforglipron is taken once daily with or without food, swallowed whole, and the UK government says it can be taken at any time of day with no food or water restrictions. That is its main practical edge over the semaglutide pill, which has strict conditions.
For the Wegovy pill, the European product page is precise: one tablet a day on an empty stomach, after at least 8 hours of fasting, then a 30-minute wait before eating, drinking or taking other medicines (EMA Wegovy page). For many people that morning routine matters more than the choice of molecule. What is established for orforglipron is the absence of meal rules in the US and UK labels; no EU label exists yet.
Orforglipron vs Zepbound or Mounjaro vs the Wegovy pill: what does the comparison say?
No trial has compared orforglipron directly with tirzepatide (Zepbound in the US, Mounjaro elsewhere) or with the Wegovy pill in obesity, so every comparison of figures is indirect, across trials with different populations, durations and methods. It gives an order of magnitude, not a reliable ranking. The table sets the published results side by side.
| Treatment (trial) | Form | Average weight loss |
|---|---|---|
| Orforglipron 36 mg (ATTAIN-1, 72 wk) | Daily tablet | −11.2% (placebo −2.1%) |
| Oral semaglutide 25 mg (OASIS 4, 64 wk) | Daily tablet, fasting | −13.6% (placebo −2.2%) |
| Semaglutide 2.4 mg (SURMOUNT-5, 72 wk) | Weekly injection | −13.7% |
| Tirzepatide (SURMOUNT-5, 72 wk) | Weekly injection | −20.2% |
For tirzepatide, SURMOUNT-5 is the only direct comparison with injectable semaglutide (−20.2% versus −13.7%), and SURMOUNT-1 measured −20.9% at the 15 mg dose against −3.1% on placebo. For oral semaglutide, Novo Nordisk reports 16.6% among people who adhere to treatment (release of 22 May 2026), versus 13.6% in the analysis of all participants. A cautious reading: on these numbers orforglipron sits below the injections and somewhat below the Wegovy pill, but the gap was never tested head to head. In type 2 diabetes, ACHIEVE-3 (Lancet, 2026) did compare orforglipron with oral semaglutide, with HbA1c falling 1.91% versus 1.47% at the higher doses, but those are other doses and another condition. For the molecules coming after, see our survodutide, retatrutide and orforglipron overview, written before Foundayo was approved.
What side effects does orforglipron cause?
Orforglipron’s side effects are those of other GLP-1 drugs: mostly gastrointestinal, and mostly mild to moderate. In ATTAIN-1, adverse events led to treatment discontinuation in 5.3% to 10.3% of participants on orforglipron versus 2.7% on placebo, so the pill is not better tolerated by design, only simpler to take.
The US label carries a boxed warning for the risk of thyroid C-cell tumours seen in rodents, as other GLP-1 drugs do, and the drug is contraindicated with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2 (US label). Whether this applies to you is a question for your prescriber, who knows your history.
What if you are already taking Zepbound, Mounjaro or Wegovy?
If you are already on Zepbound, Mounjaro or Wegovy with a result you are happy with, nothing in the published data justifies stopping an effective treatment to wait for orforglipron. There is no dose equivalence between these molecules, and a switch means a new titration with another stretch of digestive side effects.
We do not publish any injection-to-pill switching schedule: that belongs to the prescription. To prepare the conversation with your prescriber, log your weigh-ins, injections and side effects in the MounjaGO app, which turns them into a PDF report to show them. You will learn more from your own trajectory than from any trial average.
What are the limits of these data?
The orforglipron figures are averages from manufacturer-funded trials in specific populations: they do not predict your own result, and say nothing about efficacy or safety beyond 72 weeks, nor about how orforglipron performs in everyday practice. Several limits frame how to read them.
- No head-to-head trial. Nothing compares orforglipron with tirzepatide or the Wegovy pill in obesity; the table sets different trials side by side.
- Funding. ATTAIN-1 was funded by Eli Lilly, and several authors are employees.
- Doses. Trial doses (6, 12 and 36 mg) are not expressed like those on the US label; we publish no individual dose.
- European timeline. No CHMP opinion, authorisation date, price or reimbursement is established for the EU; the agenda only shows where the file stands.
- Long-term data. Stopping treatment, weight regain and safety beyond 72 weeks are not covered here.
Frequently asked questions
Is orforglipron approved in the US and UK?
Yes. The FDA approved it as Foundayo on 1 April 2026 and the UK’s MHRA authorised it on 10 August 2026. In the UK it is not currently available through the NHS while NICE evaluates it. In the EU it remains under review.
Are Foundayo and orforglipron the same thing?
Yes. Orforglipron is the molecule and Foundayo is the brand name Eli Lilly chose for the tablet approved in the US and UK. A European brand name is not known until the drug is authorised there.
Does orforglipron cause as much weight loss as Zepbound?
On published numbers, no: 11.2% at 72 weeks for the highest orforglipron dose versus 20.2% for tirzepatide in SURMOUNT-5. These come from different trials never compared head to head, so the gap is only indicative.
Can orforglipron be taken with a meal?
Yes, per the US prescribing information: the tablet is taken once daily with or without food. That is the difference from the Wegovy pill, taken fasting and 30 minutes before eating.
